Overview The Visiting Nurse Association of Florida (VNA) is a 50-year-old home health agency that is growing and hiring Speech Therapists in Lake County. As a Speech Therapist , you will ensure delivery of quality speech therapy services and provide case management for patients through comprehensive assessments and coordination of agency services. You will work with family members and local healthcare professionals to implement services to meet the needs of patients and families in their homes. You will be responsible for completing physical assessments and determining patient home care needs while working with the care team within VNA of Florida's case management model. Responsibilities Deliver speech therapy services and provide case management for patients in home settings. Perform comprehensive assessments and coordinate agency services to meet patient and family needs. Collaborate with family members and local healthcare professionals to implement care plans at home. Work with the care team to determine home care requirements and support patient goals.
