Position Overview

Primary Care Nurse Practitioner with 1+ years of nursing home or geriatric experience needed to provide primary care services at a nursing home facility in Pittsburgh, PA.

Compensation $11,500 annually

\n

Schedule Flexible scheduling! 8-10 hours per month

\n

Location Nursing home in Pittsburgh, PA

\n

Key Responsibilities

\n

\n
• Monthly CCM follow‑ups — Provide structured, chronic care check‑ins for eligible residents.
\n
• Care plan creation & updates — Develop and maintain comprehensive, patient‑centered care plans for chronic conditions.
\n
• Medication reconciliation — Review medications monthly to ensure safety, accuracy, and adherence.
\n
• Coordination with specialists — Communicate with cardiology, psychiatry, physiatry, and other specialists involved in chronic disease management.
\n
• Monitoring chronic conditions — Track symptoms, labs, vitals, and disease progression for residents with two or more chronic illnesses.
\n
• Documentation for CMS billing — Record time spent, care coordination activities, and care plan updates to meet CCM billing standards.
\n

Required Qualifications

• Active Nurse Practitioner license in Pennsylvania with Board Certification.
\n
• 1+ year of nursing home or long‑term care experience .
\n
• Strong clinical judgment and comfort working independently.
\n
