Contract | Days | Outpatient Neuro Setting (Outpatient HUB within Inpatient Rehab)

Key Responsibilities

• Complete comprehensive evaluations of assigned patients and develop individualized treatment plans with goals focused on progressing patients toward discharge within an established length of stay.
• Collaborate with an interdisciplinary team to address all barriers to discharge, including equipment trialing and ordering, patient and family training, home exercise program issuance, and coordination of continuing services.
• Manage individual, concurrent, and group treatment sessions effectively, ensuring the plan of care is communicated, implemented, and progressed throughout the length of stay.
• Complete accurate and timely medical record documentation including daily notes, goal writing, functional quality indicator assessments, letters of medical necessity, discharge documentation, and charge entry in compliance with state, federal, and payer guidelines.
• Participate in student clinical education programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations.
• Actively engage in professional development, performance improvement initiatives, and patient and community advocacy efforts.

Required Qualifications

• Active Physical Therapist licensure in good standing in the state of Kentucky.
• Adherence to the state Physical Therapy Practice Act and all applicable regulatory requirements.
• Completion of all state-mandated continuing education requirements.
• Demonstrated competency in performing job functions in accordance with the population served.
• Commitment to patient and employee safety standards, including safe patient handling, infection control, and hazard reporting.

Preferred Experience / Skills

• Prior experience in an outpatient neurological rehabilitation setting.
• Clinical rotation experience in outpatient therapy if direct experience is not available.
• Familiarity with functional quality indicators and insurance authorization processes within a rehabilitation environment.
