Description

• Assess patients in either an inpatient or outpatient context and formulate personalized treatment strategies
• Implement physical therapy techniques focused on fostering mobility, enhancing strength, improving balance, and restoring function
• Work collaboratively with nurses, physicians, case managers, and other rehabilitation experts to maximize patient outcomes
• Provide education to patients and families on therapeutic objectives, discharge considerations, and at-home exercises
• Keep thorough documentation of evaluations, treatments, and patient progress in accordance with facility and compliance regulations
• Engage in care team meetings and interdisciplinary discussions to promote coordinated care efforts

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized program

Certifications

• State licensure as a Physical Therapist
• Preferred CPR certification
• Specialized certifications (such as in neurology or geriatrics) are advantageous but not mandatory

Skills

• Relevant experience is strongly preferred
• Capability to address the needs of patients with intricate medical and mobility challenges
• Effective communication and teamwork abilities
