Neighborhood Healthcare PACE is a managed medical plan built around surrounding participants with a team of physicians, nurses, social workers, therapists and care coordinators to help them maintain good health and a good quality of life. Our goal is to keep our seniors happy and healthy at home surrounded by their family and community.
As a private, non-profit 501(C) (3) community health organization, we serve over 500k medical, dental, and behavioral health visits from more than 100,000 people annually. With two PACE centers located in Riverside County, our PACE program is positioned to serve over 650 senior participants.
ROLE OVERVIEW and PURPOSE
The PACE Rehabilitation Manager oversees the operations of the rehabilitation department, ensuring high-quality services that align with state and federal regulations. This role provides administrative supervision and leadership for the therapy team, including physical therapy, occupational therapy, and speech language pathology services. Clinical supervision is provided within the manager’s licensed scope of practice, while designated discipline-specific leads or discipline-specific evaluating therapists provide clinical oversight for services outside the manager’s licensed discipline. Schedule: Monday-Friday 8am-4:30pm.
Responsibilities
- Orients new staff to the PACE (Program of All-Inclusive Care for the Elderly) model, CMS, and regulatory requirements while ensuring compliance with PACE guidelines
- Provides ongoing coaching, mentoring, and training to promote clinical excellence and team cohesion
- Leads, coordinates, and supervises licensed therapy professionals within scope of practice (this may include disciplines of Occupational Therapy, Physical Therapy, and Speech Language Pathology) and aides, ensuring continuity and consistency of participant care planning
- Monitors staffing patterns, caseload distribution, and service coverage to ensure operational efficiency and appropriate workload balance
- Supports workforce planning, onboarding, and role development to meet evolving program needs
- Maintains competency in Interdisciplinary Team (IDT) processes and facilitates IDT meetings as directed, ensuring effective coordination of care and operational alignment
- Ensures standardization of operations within scope; identifies, reports, and addresses variances with rationale to maintain alignment with program expectations
- Ensures clinical supervision structures are clearly defined and maintained across disciplines, with appropriate delegation and oversight in accordance with scope of practice and regulatory requirements
- Assists in developing policies and procedures to ensure department best practices are followed, therapy interventions meet quality standards, and all activities are safe, culturally appropriate, and compliant with regulatory standards
- Develops and implements processes to ensure departmental continuous tracking and monitoring of the provision of rehabilitative related services
- Attends and participates in staff meetings, organizational QI activities, and assigned committees, including participant care planning, utilization reviews, ethics, grievances, and appeals
- Oversees Home Health Occupational Therapy, Physical Therapy, and Speech Language Pathology provision of services ensuring quality and timely services for participants in their homes
- Develop and manage the DME process, including ordering, storage, maintenance, and tracking, ensuring compliance with regulatory standards
- Oversee DME safety and in-home installations to enhance participant independence and safety
- Ensures delivery of quality services by overseeing the department's adherence to policies/procedures and compliance with state and federal regulatory requirements
- Ensures services provided are in a therapeutic setting and conducive to restoring dignity and self-esteem to patients
- Monitor site and department budgets to maximize cost-effectiveness of operations while ensuring high-quality care is maintained
- Provides ongoing oversight of Skilled Nursing Facility (SNF) discharges and transitions, ensuring coordination with SNF providers through regular utilization meetings to support effective care planning and continuity of care
- Completes all required training programs and participates in ongoing conference calls, webinars, and other professional development opportunities
- Partners with the Center Director and Quality and Compliance to review, investigate, and resolve participant grievances, appeals, and service concerns, ensuring timely resolution and appropriate documentation
- Provides coverage for staffing gaps, including LOA, PTO, and high-volume periods, by stepping in to support operations as needed within respective scope of license
- Maintains the confidentiality of all company procedures and participant information in conformance with HIPAA principles
- Keeps abreast of all regulations and standards to ensure compliance with orders or directives issued by duly constituted government and regulatory agencies, such as PACE, CMS, and DHCS
- Functions at highest level according to credentials and competency
- An Occupational Therapist or Physical Therapist, within the respective discipline scope:
- Perform comprehensive assessments of participants to determine physical abilities, mobility needs, and other therapeutic requirements
- Develop, implement, and update individualized care plans aimed at improving functionality, enhancing independence, and promoting optimal health
- A Certified Occupational Therapy Assistant or Physical Therapy Assistant, within the respective discipline scope:
- Implement and update progress of individualized care plans aimed at improving functionality, enhancing independence, and promoting optimal health
- An Occupational Therapist or Physical Therapist, within the respective discipline scope:
- Guides participants and caregivers in the use of adaptive equipment, durable medical equipment, and functional techniques to improve safe daily function
- Continuously evaluates the effectiveness of treatment plans and prompts the modification of therapeutic approaches, as necessary, to achieve the best possible outcomes
- Participates in site visits to participants’ homes, hospitals, or other facilities as required by the interdisciplinary team (IDT)
- Maintains accurate records, including treatment plans, progress notes, and discharge summaries, in compliance with organizational policies and regulatory requirements
- Leads, coaches, and develops rehabilitation staff, including hiring, onboarding, performance management, and disciplinary actions in coordination with Human Resources
- Performs other duties as assigned
Qualifications
Education/Experience
- Physical/Occupational Therapist
- Graduation from an accredited Occupational Therapy or Physical Therapy program is required
- Must be a licensed Occupational Therapist or Physical Therapist in the State of California
- 5 years of experience as an OT or PT required
- COTA/PTA
- Graduation from an accredited Occupational Therapy Assistant or Physical Therapy Assistant program is required
- California- licensed therapy aide required
- 5 years of experience working as a COTA or PTA required
- 2 years of progressive leadership experience preferred
- Completion of LEAP Program preferred (Internals Only)
- Previous PACE experience and/or experience with frail and elderly population preferred
- Current Basic Life Support (BLS) certification through an American Heart Association (AHA)-approved source is required upon hire and must be maintained as a condition of employment.
- AHA-approved courses include an in-person, hands-on skills check with a certified instructor using a mannequin to demonstrate CPR and emergency response techniques
- Online-only BLS courses without a live skills check do not meet this requirement
- In addition, current CPR and First Aid certification are required for all PACE employees in accordance with CMS and DHCS requirements. First Aid training will be provided at onboarding and must be renewed annually

- Ability to interpret a variety of instructions in written, oral, diagram, or schedule form
- Knowledgeable about and experience with providing therapy to geriatric participants
- Ability to apply analytical thinking and problem-solving to resolve issues
- Ability to travel between sites in multiple regions
- Ability to successfully manage multiple tasks simultaneously
- Excellent project management, planning, and organizational ability
- Ability to work as part of a team as well as independently
- Ability to work with highly confidential information in a professional and ethical manner

Pay range for OT/PT: $124,100 - $191,000 annually, depending on experience.
Pay range for COTA/PTA: $87,500-$127,700 annually, depending on experience.

While this range represents the broader classification of the role, actual compensation will be based on several factors, including but not limited to the candidate’s overall knowledge, skills, and experience, market data and industry benchmarks, internal equity within the organization, Budgetary considerations and organizational needs. As a result, placement within the range is not guaranteed, and the full pay grade range may not be utilized.
