About the Role:

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Charter Senior Living of Chattanooga. As an LPN, you will play a vital role in providing high-quality patient care to our seniors. LPN will be responsible for managing patient care, administering medications, and providing emotional support to our residents.

Responsibilities:

• Provide direct patient care to residents, including administering medications, wound care, and treatments
• Monitor vital signs, report changes to the healthcare team, and take appropriate action
• Develop and implement individualized care plans in collaboration with the healthcare team
• Maintain accurate and detailed records of patient care and medication administration
• Participate in Quality Assurance Program and adhere to policies and procedures
• Collaborate with other healthcare professionals to ensure seamless and efficient care
• LPN will also be responsible for performing other duties as assigned, including assisting with activities of daily living (ADLs) and contributing to the overall well-being of our residents.

Requirements:

• Current LPN license in the state of Tennessee
• Current CPR certification
• Ability to pass a background check
