Social Work Care Manager: Discharge & Care Coordination

McLaren Health Care

McLaren Health Care in Detroit, Michigan is seeking a licensed Master’s Social Worker to provide assessment, intervention, and discharge planning services. This role includes addressing patients’ psychosocial needs and connecting them with community resources. The ideal candidate should have LMSW certification, experience in acute hospital care, and knowledge of case management practices.
