Responsibilities
- Plan and conduct segments of treatment programs to restore and improve physical function, and support social and mental well-being as part of an interdisciplinary team
- Perform comprehensive evaluations, establish individualized treatment plans, and deliver therapy interventions using established protocols
- Develop and communicate discharge and follow-up plans; document progress and outcomes in the chart
- Supervise assigned assistants and coordinate care with nursing and other rehabilitation staff
- Maintain comprehensive, accurate clinical documentation within established deadlines
- Demonstrate proficiency with the Epic EMR system and participate in workflow improvements as needed

Requirements
- Doctor of Physical Therapy (DPT) degree required; only if the license was obtained on or after January 2016, or foreign equivalent
- Physical Therapy licensure in the state of practice required
- Minimum two years’ Physical Therapy experience preferred
- Current CPR/BLS certification (per AH policy) required
- Must have EPIC experience
- Minimum 40-hour-per-week availability; ability to work in outpatient and inpatient settings
- A current nursing license per the state of assignment
