As a Physical Therapist with our team, you will have the opportunity to make a difference in the lives of our residents by helping them achieve their optimal physical performance. Our mission is to meet the physical, emotional, and spiritual needs of our residents in a compassionate environment that promotes dignity and respect. Responsibilities Develop individualized plans of care to facilitate optimal performance using standardized tests and measures as evidence for improvement Identify needs and make referrals to other disciplines to address the comprehensive needs of patients Monitor patients\' progress and adjust treatments accordingly Provide supervision for Physical Therapy Assistants, students, and support personnel in accordance with state license requirements and professional standards Perform other duties as assigned to provide quality care to residents Qualifications Experience providing physical therapy to patients in a skilled nursing facility Graduated from an accredited physical therapy program Current Physical Therapy license for the state of Delaware Current CPR certificate Good organizational, time management, and communication skills How to Apply If you are a caring and compassionate Physical Therapist looking to join a team that is dedicated to providing exceptional care to our residents, please apply to this role.
