Social Worker Outpatient

Full time (Monday - Friday, days), no nights, no on-call, no weekends, no holidays. Social Worker Outpatient - Licensed Perform evaluations and services to patients of various age groups, collect data and render a professional opinion about treatment and care. What will you do in this role: Perform vocational rehabilitation assessment in a timely manner; dictate interdisciplinary report to clinical record within the required time frames. Initiate and discharge documents within required timeframe. Document therapy notes on the clinical record. Formulate and review initial and comprehensive treatment plans within the required time frame. Identify and document patient care needs. Work closely with the interdisciplinary team to assure appropriate patient care; know, understand, maintain and monitor patients’ rights. Provide patient/family emotional support and education. Meet in-service requirements including mandatory education. Perform other duties as required; read, understand, and adhere to all Physician Services policies and procedures. Practice and adhere to the Code of Conduct and Mission and Value Statement. Qualifications Education Post Graduate degree. Experience Minimum of 3-5 years’ experience. Certification / License A Clinical Mental Health Counselor license or Independent Clinical Social Worker license required in accordance with state regulations.
