Job Summary:

We are seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to join our team at Avir at French Home in San Antonio, TX. Responsibilities:

• Provide direct patient care to residents in accordance with established policies and procedures

• Administer medications, treatments, and therapies as prescribed by physicians

• Monitor and report changes in resident condition, including vital signs and laboratory results

• Implement and update care plans in collaboration with the interdisciplinary care team

• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

• Maintain accurate and detailed records of patient care and treatment

• Collaborate with other healthcare professionals to ensure seamless care transitions and coordinated patient care

Requirements:

• Current LVN licensure in the state of Texas

• 1+ years of experience as an LVN in Long Term Care

• Strong clinical skills and knowledge of geriatric care
