Are you passionate about enhancing patient care and ensuring smooth transitions for individuals returning home? Langdon Place of Dover is seeking a dedicated and experienced Director of Discharge Planning to lead our efforts in providing exceptional discharge services for our residents.
Key Responsibilities:
Oversee the discharge planning process, ensuring a seamless transition from our facility to home or other care settings.
Collaborate with interdisciplinary teams to assess patient needs and develop individualized discharge plans.
Foster strong relationships with community resources and services to support patients post-discharge.
Monitor and evaluate discharge outcomes to continuously improve processes and patient satisfaction.
Provide leadership and guidance to discharge planning staff, promoting professional development and teamwork.
Qualifications:
Bachelor’s degree in Nursing, Social Work, Healthcare Administration, or a related field.
Skilled nursing experience preferred.
Proven experience in discharge planning or case management, ideally in a healthcare or rehabilitation setting.
Strong understanding of healthcare regulations and community resources.
Excellent communication and interpersonal skills.
Compassionate and patient-centered approach to care.
