In this role, you will provide compassionate, resident-centered nursing support in an assisted living and memory care environment. You will administer medications and treatments, monitor residents for changes in condition, support individualized service plans, and communicate with residents, families, providers, pharmacies, and community team members.

What You'll Do

• Administer medications and treatments according to provider orders, nursing direction, applicable standards, and community procedures
• Observe residents for changes in physical condition, cognition, behavior, mood, appetite, mobility, and daily functioning
• Obtain and document vital signs and other assigned resident health information
• Provide supportive and restorative nursing care within the New York LPN scope of practice
• Collect, record, and report resident health information to the supervising nurse or authorized provider
• Promptly report significant changes in resident condition
• Carry out nursing interventions, treatments, and instructions established by the supervising nurse or provider
• Participate in resident assessments and service-plan reviews under appropriate clinical direction
• Assist with implementing and maintaining individualized resident service plans
• Receive, review, and carry out provider orders within the LPN’s scope of practice and community procedures
• Coordinate medication refills and communicate with pharmacies and providers
• Maintain accurate electronic and written documentation of medications, treatments, observations, incidents, and nursing services
• Perform medication-cart, treatment-record, and documentation audits as assigned
• Support resident admissions, hospital returns, transitions, and changes in condition
• Provide health teaching and wellness counseling within the LPN’s training, competency, and scope of practice
• Promote hydration, nutrition, infection prevention, medication safety, fall prevention, and overall wellness
• Respond promptly to resident emergencies and urgent needs
• Perform CPR or Basic Life Support when trained, certified, and required
• Communicate with families and providers regarding resident needs and changes as directed
• Participate in resident-care meetings and Wellness team communication
• Provide day-to-day direction and support to resident-care team members as assigned
• Reinforce resident service plans, care instructions, and community procedures
• Monitor assigned resident-care activities and promptly report concerns to the supervising nurse
• Assist with staff training, competency reviews, audits, and required in-service education
• Maintain resident privacy, dignity, rights, and confidentiality
• Complete additional nursing and Wellness duties within the LPN’s lawful scope of practice

What We're Looking For

• Current and valid New York State LPN license
• License must be active and in good standing
• Knowledge of practical nursing standards, medication administration, infection prevention, and resident safety
• Strong observation and resident-monitoring skills
• Ability to recognize, document, and report changes in resident condition
• Ability to work under appropriate clinical direction and seek guidance when nursing assessment or independent professional judgment is required
• Strong documentation, organization, and time-management skills
• Comfortable using electronic health records or resident-care documentation systems
• Ability to communicate professionally and compassionately with residents, families, providers, pharmacies, and coworkers
• Ability to communicate clearly and effectively in English, verbally and in writing, as required to understand provider orders, document nursing services, administer medications safely, report changes in condition, and respond during emergencies
• Ability to provide appropriate direction and support to resident-care staff
• Ability to work independently within the assigned role while recognizing when a concern must be escalated
• Current CPR or Basic Life Support certification, if required by the community
• Previous assisted living, memory care, long-term care, home care, or senior-care nursing experience is preferred
• Previous experience working with older adults is preferred
• Dependable attendance and punctuality
• Ability to stand, walk, bend, reach, and remain active throughout the workday
• Ability to safely lift, transfer, push, or pull up to (physical requirement) pounds with assistance or available equipment
• Ability to successfully complete required background checks, health screenings, and employment requirements

New York Nursing Requirements

• Must maintain an active New York State LPN license throughout employment
• All nursing services must be performed within the New York LPN scope of practice
• The LPN provides nursing care under the direction of a Registered Nurse, physician, or other licensed provider authorized by New York law
• The LPN may collect resident information, provide supportive and restorative care, administer medications and treatments, and report resident outcomes
• Comprehensive nursing assessment, nursing diagnosis, and responsibilities requiring independent professional nursing judgment must be completed by the appropriately authorized licensed professional
• The LPN must promptly report significant changes, unstable conditions, and concerns requiring additional clinical judgment
• Duties must be consistent with the resident’s condition, provider orders, the LPN’s education and demonstrated competency, clinical direction, and community procedures
• The LPN may provide direction and follow-up to resident-care staff but may not assign duties outside the employee’s training, qualifications, or permitted role
• Medication administration and nursing documentation must comply with provider orders, applicable New York requirements, and community procedures
• Specialized procedures may only be performed when legally permitted, specifically assigned, and supported by appropriate education, competency, and clinical oversight
