Be prepared to provide a current CV, Michigan license, board documentation, DEA, and a recent NPDB self-query. The facility also requires: Current-season influenza vaccination COVID-19 documentation or declination TB testing within 12 months plus questionnaire Hepatitis B, MMR, and Varicella vaccine series or quantitative titers Tdap within the last 10 years Travel & Logistics
