Primary Care Physician

We are looking for a primary care physician to join our team full-time at Conviva Primary Care. This role is ideal for a compassionate, experienced clinician who excels in a collaborative, patient-centered environment and is committed to improving outcomes for adult and geriatric populations.

Main Responsibilities

• Deliver comprehensive outpatient care to adult and senior patients.
• Maintain accurate medical records and documentation.
• Diagnose and manage moderately to complex medical conditions.
• Coordinate referrals and collaborate with specialists.
• Participate in clinical quality improvement projects.
• Work with interdisciplinary teams to ensure holistic care.
• Contribute to projects and innovations in care delivery.

Required Qualifications

• MD or DO from an accredited medical school.
• Active, unrestricted medical license in the state of practice.
• Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine (or board eligible).
• If selected for this role, you will be required to be screened for TB.

Must be willing to participate in on-call coverage by phone.

Preferred Qualifications

• Two years' experience in value-based care or managing high-acuity geriatric patients.

Scheduled Weekly Hours: 40

Pay Range: $255,000 - $298,500 per year. This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance. Associates may receive a bonus for the referral of external candidates to this requisition, provided that all other eligibility requirements are met.
