We Are Hiring Location: Nashville, TN Facility: Ascension Saint Thomas Department: Population Health Schedule: Full-time | Days What You Will Do Navigate & Advocate: Serve as a compassionate guide for patients throughout their entire healthcare journey, ensuring seamless continuity of care. Bridge & Collaborate: Act as the vital liaison connecting physicians, administration, community organizations, and insurers to keep everyone aligned. Support the Underserved: Champion health equity by securing critical referrals and follow-up care resources for uninsured and underinsured patients. Analyze & Improve: Track patient outcomes and compile data to spearhead educational initiatives and drive meaningful process improvements. What You Will Need Licensure / Certification / Registration: Licensed Registered Nurse credentialed from the Tennessee Board of Nursing OR current home state license for multi-state license recognition, “Compact State,” obtained prior to hire date or job transfer date required. BLS Provider preferred. American Heart Association or American Red Cross accepted. Education: Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.
