As a Physical Therapist Assistant, you will play a vital role in providing skilled therapy services directly in patients' homes. You will work under the direction of licensed Physical Therapists to carry out treatment plans and monitor patient progress. Your responsibilities will include:

• Providing skilled therapy services in patient homes
• Carrying out established treatment plans involving therapeutic exercise, gait training, balance activities, and functional mobility intervention
• Monitoring and communicating patient progress and changes to supervising PT
• Encouraging patient participation and reinforcing goals
• Completing timely, accurate documentation in EMR
• Collaborating with therapists, nurses, caregivers, and other care team members

To be successful in this role, you should have:

• Current Colorado Physical Therapist Assistant (PTA) license in good standing
• Graduate of an accredited Physical Therapist Assistant program
• Valid driver's license, reliable transportation, and auto insurance
• Strong communication and documentation skills
• Ability to work independently and adapt professionally in a home-based setting
• Need 1 year experience in setting
