RN LPN Resident Care Manager

Job TitleJob responsibilities:Daily rounds to monitor resident condition and staff performanceCoach and/or counsel staff providing care on your unitReview of documentation to ensure compliance and accurate chartingAssist floor staff with emergencies, changes of condition and MD calls as neededParticipates in Care Conferences and quarterly reviews.Ensures weights are obtained per policy, reviewed and referrals made as appropriateParticipate in wound rounds weekly and assist with documentation as neededParticipate in psychotropic review monthly with follow up on team recommendationsTrack and ensure consultant recommendations are completed and results documentedAssist in the admission and discharge process for residentsReview/investigate Incident reports utilizing root cause analysis (the 5 why's) to identify cause and implement appropriate preventative measuresOn-call rotation (approx every 5 weeks)Performs duties as a floor nurse if staffing crisis occurs.Job requirements:WA state RN License in good standingCPR certificationExcellent interpersonal skillsAble to work with time constraints and stressLTC experience preferredWork Remotely: NoJob Type: Full-timePay: $89440 - $118000 per yearBenefits:401(k)Dental insuranceHealth insurancePaid time offRetirement planVision insuranceMedical Specialty: GeriatricsSchedule:8 hour shiftHolidaysMonday to FridayOn callWeekends as neededExperience:Nursing: 2 years (Required)Case management: 2 years (Required)License/Certification:RN(Required)Work Location: In person
