Seven Healthcare are seeking an experienced Physical Therapist for a travel assignment in New York. This role involves providing case management and direct patient care in a home setting, specifically supporting the Monroe County team. Candidates must have Oasis experience and will work within a team to administer comprehensive physical therapy modalities.

Key Responsibilities of the Physical Therapist

• Provide case management and direct patient care within a team environment
• Administer physical therapy modalities in home settings
• Assist physicians in evaluating levels of function and developing care plans
• Prepare clinical and progress notes

Requirements of the Physical Therapist

• B.S., M.S., or D.P.T. degree in Physical Therapy
• Current New York State License in Physical Therapy
• BLS certification (AHA only)
• Minimum of one year experience in Long Term Care/Home Care
• Recent Home Care experience (within the last year)
• Oasis experience is mandatory
