Baylor Licensed Practical Nurse (LPN) - Brackenridge, Pennsylvania We are seeking a dedicated and compassionate Baylor Licensed Practical Nurse (LPN) to join our team at Eden Nursing & Rehabilitation (formally known as Platinum Ridge). As a valued member of our team, you will play a vital role in providing high-quality patient care to our residents, while upholding our commitment to excellence and compassion. Shift: Full Time, Saturday & Sunday 7am-11pm. Rate of Pay : $28-$32/hour About the Role: As a Baylor Licensed Practical Nurse (LPN), you will be responsible for providing evidence-based nursing care to our residents, under the supervision of our healthcare team. You will assess patient needs, develop and implement individualized care plans, and coordinate with other healthcare professionals to ensure optimal patient outcomes. Your expertise will also be utilized in mentoring and educating our Care Assistants and other nursing staff. Responsibilities: Provide direct patient care to our residents, including assessing, planning, implementing, and evaluating care plans Develop and maintain accurate and up-to-date patient records Collaborate with the interdisciplinary team to develop and implement individualized care plans Administer medications and treatments as ordered by physicians Perform routine vital signs and monitor patient conditions Provide patient education and support to patients and their families Participate in quality improvement initiatives to improve patient outcomes and satisfaction Qualifications: Must be a licensed Practical Nurse in the state of Pennsylvania Strong communication and interpersonal skills Ability to work effectively as part of a team Compassion and commitment to delivering high-quality patient care What We Offer: Competative Pay Same Day Pay Holiday Pay PTO Health Insurance Dental Insurance Vision Insruance 401K Life Insurance Bereavement Leave
