Schedule/Hours: Flexible daytime hours tailored to patient and nurse availability Qualifications: Current and unencumbered RN license Minimum of one year of recent experience in home health or a related acute care setting Thorough understanding of OASIS documentation and home health best practices Valid drivers license and reliable transportation Proficiency with updating electronic health records Outstanding clinical assessment and communication skills Demonstrated ability to work independently and manage a varied caseload Key Responsibilities: Perform comprehensive in-home patient assessments and ongoing evaluations Administer medications, wound care, IV therapy, and related skilled nursing interventions Develop and adjust individualized care plans in collaboration with physicians and interdisciplinary teams Educate patients and caregivers on self-care, treatment plans, and health management Accurately document all care and observations within required time frames Maintain compliance with all local, state, and federal regulations This contract position provides an opportunity to make a meaningful impact through direct patient interaction while enjoying the variety and flexibility that home health offers.
