Job Title Physical Therapist Location Boston, MA | Skilled Nursing Facility Schedule / Job Type Contract | Days | 13-Week Assignment Key Responsibilities
- Evaluate and treat patients in a skilled nursing facility setting to restore function and improve quality of life
- Develop and implement individualized physical therapy treatment plans based on patient needs and goals
- Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
- Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
- Educate patients and caregivers on therapeutic exercises and home safety strategies
- Active Physical Therapist (PT) license in the state of Massachusetts
- Graduate of an accredited Physical Therapy program
- Prior experience working in a skilled nursing or long-term care environment
- Ability to work day shift hours
- Experience with MDS assessments and Medicare documentation requirements
- Familiarity with electronic medical records (EMR) systems
- Strong communication and patient-centered care skills
