Nurse Practitioner - PRN

Join a team providing in-home health assessments to improve patient outcomes and manage care gaps. Responsibilities Include Conduct comprehensive assessments of adult and geriatric patients, including medical history, diagnosis, treatment, and social factors. Administer and interpret point-of-care and lab tests. Collaborate with physicians, case managers, social workers, and caregivers to develop and follow up on care plans. Provide education and referrals as needed, documenting all interactions per protocols. Perform services at members' residences, including homes and Skilled Nursing Facilities. Qualifications And Skills Master’s Degree or equivalent, with NP licensure and prescriptive authority in practice state. Board certification in Adult, Adult Gerontology, Family, Emergency, or Acute Care specialties. Current BLS, ACLS, or CPR certification. Strong communication, computer skills, and a valid driver’s license with insurance. Experience preferred but new graduates encouraged to apply.
