MSW Behavioral Health Specialist - IMT - Bronx, Brooklyn, Queens, Manhattan

Work Setting

Community-based field role, involving travel across New York City boroughs utilizing public transportation.

Core Duties

•Engage individuals in the community (streets, shelters, transitional housing, homes, and other natural settings)
•Conduct face-to-face outreach and ongoing engagement with hard-to-reach populations
•Build trust using trauma-informed, person-centered, and harm reduction approaches
•Use motivational interviewing and de-escalation techniques to support engagement and stabilization
•Provide ongoing psychosocial support and recovery-oriented interventions
•Assist clients in managing serious mental illness (e.g., schizophrenia, bipolar disorder, depression)
•Support clients with harm reduction strategies, including overdose prevention education and participation in naloxone (Narcan) training/distribution under clinical supervision
•Assist with practical needs and system navigation (housing applications, benefits enrollment, connection to treatment)
•Coordinate care with a multidisciplinary team
•Complete assessments, documentation, and progress notes within required timelines (typically 48 hours)
•Participate in team-based planning and shared caseload responsibility
•Utilize computer systems for document

Required Qualifications

•Master's Degree in Social Work, Psychology, Mental Health Counseling, or related field
•Minimum of two (2) years' experience with individuals with a mental health condition and co-occurring substance use disorder (life experience accepted)
•Ability to work flexible hours including mornings, evenings, and weekends
•Ability to prioritize and meet deadlines
•Creative and flexible
•Initiative and responsibility for follow-through
•Ability to work in a multi-disciplinary team setting
•Skills in conflict mediation/negotiation and assertive problem-solving
•Comfortable using public transportation and working in non-traditional environments
•Computer proficiency in Windows operating systems and programs (MS Word, Excel, PowerPoint) and learning new electronic systems
•Fingerprint clearance by the New York State Justice Center
•Understanding and commitment to Community Access's philosophy and mission.
