About this role:

As aPhysicianin LifestyleMedicine,you will work with a team of healthcare professionals committed to providing quality care andanexcellentpatient experience.In thisrole,you willprovide clinical services topatientsforpreventing, treating, and managing chronic diseases andimproving quality of life.

• This position includes a $15,000 sign-on bonus,and up to $20,000 in relocationassistance.

• You will practice atour Intermountain Health Live Well Center - Park City, UT

• Experienced Lifestyle Medicine Physicians and New Graduates with Wellness Certification will be considered

Whatyoullbring:

• MD or DO degreefrom a fully accredited medical or osteopathic school of medicine

• Residency traininginthedesignateddiscipline

• Board certification or eligibility inthe designated discipline

• Lifestyle / Wellness Certification

• Active, unobstructedUtahmedicallicense, or the ability to obtain one

• Full prescriptive authority and an active DEA, or the ability to obtain one

• Ability to successfully complete Intermountain Healths credentialing process
