This high-touch role focuses on comprehensive care coordination for clients with complex medical and social needs, emphasizing relationship-building, advocacy, and personalized care planning. The successful candidate will enjoy autonomy, meaningful client interactions, and a collaborative team environment. Requirements Active RN license in Texas or eligibility via reciprocity. Bachelor of Science in Nursing (BSN) or higher preferred, Associates (ADN) is acceptable. Minimum of 5 years of clinical nursing experience. Residency within the Austin metro area - as far south as San Marcos. Valid driver’s license and reliable vehicle. Responsibilities Conduct client assessments and develop care plans.
