This high-touch role focuses on comprehensive care coordination for clients with complex medical and social needs, emphasizing relationship‑building, advocacy, and personalized care planning. The successful candidate will enjoy autonomy, meaningful client interactions, and a collaborative team environment. Requirements Active RN license in Texas or eligibility via reciprocity. Bachelor of Science in Nursing (BSN) or higher preferred, Associates (ADN) is acceptable. Minimum of 5 years of clinical nursing experience. Residency within the Austin metro area - as far south as San Marcos. Valid driver’s license and reliable vehicle. Responsibilities Conduct client assessments and develop care plans.
