The Referral Coordinator acts as the central communication hub for the entire referral-to-placement lifecycle across DCBS, SCL, MPW, iRise, and Child Waiver Services. This role manages clinical packets, synthesizes sensitive data for administrative review, and facilitates engagement meetings to ensure participants, families, case managers, and state agencies experience a seamless and transparent transition into services.

The Access to Care Coordinator plays a key role in expanding awareness, access, and utilization of disability-related services across the state of Kentucky. This position focuses on building relationships with community partners, distributing informational materials, and representing the organization in public-facing environments. The Coordinator helps ensure individuals with disabilities, their families, and care providers know how to access high-quality support services. This role blends outreach, education, and business development to strengthen the organization’s presence statewide.

Requirements:

 

Education & Training and/or Experience:

· High School Diploma or equivalent required. A Bachelor’s or associate’s degree in healthcare administration, Business, or a related field is highly preferred.

· Minimum 1 year of experience working specifically with Kentucky 1915(c) waivers (Supports for Community Living, Michelle P. Waiver, etc.).

· Proven ability to write "Person-Centered" notes and maintain files that withstand state regulatory reviews.

· Ability to navigate complex family dynamics and advocate for individuals with intellectual or developmental disabilities (I/DD).

· Strong communication and interpersonal skills; comfortable engaging with diverse community partners.

· Ability to travel throughout Kentucky for outreach and relationship-building.

· Self-motivated, organized, and able to work independently.

· Strong computer skills/knowledge in Microsoft Excel, and spreadsheet management.

· Strong organizational skills.
