PA/NP Urgent Care

The Chinle Comprehensive Health Care Facility (CCHCF) is based in Chinle, Arizona (Northeast Arizona near Canyon De Chelly National Monument). The CCHCF is a 60 bed hospital which serves as the health care hub for the region. The medical staff includes Family Physicians, Internists, Pediatricians, General Surgeons, OB/GYN's, Anesthesiologists, and a Psychiatrist. In addition to routine outpatient and inpatient primary care, services available to our patients include: Adult Intensive Care, General Surgery (including laparoscopic surgery), routine and operative Obstetrics, and 24-Hour Emergency Room Services. Health care services are provided to approximately 37,000 active users. Strong Navajo cultural traditions exist within the community, offering an opportunity to learn the Navajo language, or to learn about traditional Navajo medicine. The service unit is located on the Colorado Plateau with excellent opportunities for photography, hiking, running, road biking, cross country skiing, and mountain biking. CCHCF provides medical care for approximately 35,000 Navajo. Serving a rural area, many of our patients travel 100 miles round trip to receive care and do not have electricity or running water in their homes. In addition, most of our elderly patients speak only Navajo, and live according to traditional Navajo cultural practices. The CCHCF and Ambulatory Care Center is a 60-bed inpatient hospital and outpatient facility. Services offered include: Adult Inpatient, Labor & Delivery, Pharmacy, Adult Intensive Care, Mental Health, Physical Therapy, Anesthesiology, Nurse Phone Triage Services, Podiatry, Audiology, OB/GYN & Midwifery, Public Health, Dental, Occupational Therapy, Radiology, Emergency Medicine, Optometry, School Health, General Surgery, Outpatient Primary Care, Speech Pathology, Lab, Pediatric Inpatient, Women's Health

Urgent Care/ ER Department

Care for approximately 30,000 patients per year. The ER Department consists of nine (9) beds and emergency care services are provided on a 24-hour basis for adults, adolescent children and infants. The Urgent Care Department operates from 8 am - 8 pm, Monday through Sunday including Holidays. The Urgent Care Department consists of six (6) beds and is staffed by mid-level providers (Physician Assistants or Nurse Practitioners) who see patients that present for acute care and who have been triaged as lower acuity.

Responsibilities

PA/NP will provide diagnostic, preventive, and therapeutic health services to eligible patients and family members in the walk-in outpatient clinics or field clinic. The following list is not comprehensive:

• Secure a health and development history from the patient or parent;
• Record findings; make critical evaluations;
• Person or request special screening and developmental tests;
• Order laboratory tests and interpret the results;
• Discriminate between normal and abnormal findings;
• Make decisions with physicians concerning the medical care needs of patients;
• Provide direct medical care in the presence of illness or disability in order to maintain life, provide comfort, reduce distress and enhance coping ability;
• Provide surveillance over medical practices applied to stabilize chronically ill persons;
• Adjust treatment within established standing orders recognizing when to refer the patient to a physician or other health team members;
• Work collaboratively with physicians in the management of selected complex medical problems;
• When clinically engaged with a medically complex patient present the case to the physician clinic consultant for the day for proper treatment and disposition;
• Consult the clinic physician consultant or on-call hospitalist for patients assessed and deemed to require admission to the hospital;
• Participate with other health professionals and agencies involved in providing health care and services required to include immunization and disease control, environmental health and health education;
• Provide emergency or crisis intervention in order to stabilize a patient for transport to a higher-level facility;
• Provide medical care and preventive services to healthy individuals,
• Evaluate total health care needs of patients and develops plans to meet these needs;
• Required information for use in program operation, quality control and other purposes (all medical record information-generated in the performance of this contract will remain the property and subject to the exclusive control of the Indian Health Service);
• Participate in certain medical staff committee meetings as required;
• Comply with all client facility infection control and safety procedures, practices and standards;
• Meet all applicable Joint Commission on Accreditation of Healthcare Organization (JCAHO) and Centers for Medicare / Medicaid Services (CMS) standards during the performance of this contract, and must be in compliance with the Service Unit's Medical Staff Bylaws;
• Be eligible for enrollment in the service unit's Medicare part B reassignment program;
• And respect the basic rights of patients, to include but not be limited to personal dignity.

Requirements

• Must hold an active, current, and unrestricted (unflagged) state nurse practitioner (NP), or physician assistant (PA) license from any of the Fifty (50) United States or the Commonwealth of Puerto Rico.
• Must have a valid, unrestricted DEA with schedules II-V
• Cannot be under exclusions or sanctions from the Medicare / Medicaid programs.
• Must have received and will maintain national certification in Family Practice throughout the duration of this contract' period of performance.
• Must fulfill all the requirements for appointment to the Medical Staff and for the granting of privileges, in accordance with Client policies and with the Pine Ridge Service Unit Medical Staff By-Laws.
