Mental Health Case Manager/CBRS

Overview Job Title: Community Based Rehabilitation Provider This position will start part time and will grow as our caseload grows. The position also requires a Bachelors Degree. Objective To provide community based rehabilitation services to participants who are recovering from a severe and persistent mental illness that promotes recovery, assists the member to integrate with his/her community, and provides services aimed at helping the member improve his/her quality of life. Job Description A Community Based Rehabilitation Provider helps participants develop skills needed to increase their capacity to thrive in their home, employment, school or social environments. Services target skills for adults and youth that have been lost due to the symptoms of a behavioral health condition. Services vary in intensity, frequency, and duration in order to support participants in managing functional difficulties, or to otherwise realize recovery goals. A good rehabilitation provider should have excellent communication skills, problem solving skills, compassion, time management and organizational skills. Minimum Qualifications Bachelor's degree in Psychology, Social Work, Sociology, Family/Consumer Sciences, Child Development, Substance Abuse, mental health, human services, Education, Criminology, Counseling, and Therapy Fields Current background check Knowledge of community resources and counseling/social work practices with high risk populations. Experience working with persons in crisis. Good documentation skills. Excellent written and verbal communication skills, ability to establish rapport. Ability to motivate others towards achieving goals. Ability to work independently with strong sense of focus, task-oriented, non-judgmental, open personal qualities, clear sense of boundaries. A strong sense of and respect for confidentiality involving both participants and fellow employees. Ability to work in a variety of settings with culturally-diverse families and communities with the ability to be culturally sensitive and appropriate. Ability to legally operate a motor vehicle and provide own transportation. Flexible scheduling required. Job Duties/Responsibilities To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Entering, storing, or maintaining information in written or electronic form. Keeping up-to-date information and applying new knowledge to your job. Maintaining strict adherence to HIPAA standards and ethical conduct. Communicating with people outside the organization, representing the organization within one\'s job scope to vendors, the public, and other external sources. This information can be exchanged in person, in writing, or by telephone or e-mail. Provide treatment. Partner with providers to improve participant outcomes. Translating or explaining what information means and how it can be used. Encouraging and building mutual trust, respect, and cooperation among team members. Performing day-to-day tasks. As reports or other documents are received, review the document, document the review by initialing and dating the document, and place the document in the participant\'s file. If there is any follow up needed or unusual incidents noted, document observation in the participant\'s case progress notes. Adhere to agency policy, procedures and the professional code of ethics. Physical Demands & Work Environment Regularly required to communicate in person or by telephone. Must be able to travel to participant locations. A high degree of stress is possible when working with individuals that suffer from a serious mental illness. The position does not normally involve exposure to blood, body fluids or tissues; but it may require performing unplanned tasks where exposure may occur.
