Nephrology Physician Assistant

We are seeking a Nephrology Advanced Practice Provider (APP) to join Horizon Family Medical Group in Goshen, New York. This is an outstanding opportunity for a clinician passionate about providing high-quality kidney care in a collaborative, patient-focused environment.

Clinical Responsibilities:

Evaluate and manage patients with kidney disease, hypertension, and electrolyte imbalances
Develop and implement individualized treatment plans, including medication management, dialysis support, and patient education
Collaborate with nephrologists and multidisciplinary teams to provide coordinated patient care
Educate patients and families about diagnoses, treatment options, and lifestyle modifications
Maintain accurate and up-to-date electronic medical records (EMR)
Participate in quality improvement and patient safety initiatives

Required Qualifications:

NP, PA, or other APP with experience in Nephrology
Active license to practice in New York
Proven experience managing complex nephrology patients
Strong clinical and diagnostic skills
Excellent communication and patient education abilities
