Registered Nurse RN Case Manager

RN Case ManagerLocation: Florence, AL - Hybrid/Remote (ESKD/CKD)This is a hybrid/remote, field based role requiring a dedicated home office and the ability to travel to meet business and patients needs, including team and nephrology partner meetings. This position will support both ESKD and CKD patients. It is hybrid position and requires the RN Case Manager to be in the field 2-3 days per week in an assigned territory - the remaining workdays completed from a home office. Coverage includes but not limited to Florence, Hoover, Bessemer, Anniston, Russellville, Rainbow City, and Sheffield and surrounding areas. Territory assignments may change cased on business needs.Schedule: Monday - FridayFlexibility is required to accommodate patient needs and nephrology partner availability.Employment Type: Full-TimeThis position functions autonomously and in collaboration with all members of the healthcare team to coordinate and facilitate quality, cost-effective care while minimizing fragmentation of the healthcare delivery system for ESRD and CKD patients. This position provides coordination of care between the patient, family, physician, provider and care teams, the community, and Regional Operations Manager.Qualifications for the Shining Star for our DaVita IKC Registered Nurse are:A.D.N degree from accredited school of nursing required; B.S.N preferred; three-year diploma from accredited diploma program may be substituted for nursing degreeCurrent Registered Nurse (RN) license in the state of practice requiredContinuing education credits maintained as required by state of practice requiredMinimum of five (5) years' experience in clinical nursing requiredMinimum of three (3) years' experience in renal nursing preferredCase management experience is required. In lieu of case management experience, 3 or more years of experience assessing patient needs, developing and managing individualized care plans, coordinating services across interdisciplinary teams, and supporting patients through complex healthcare transitions is requiredCertified Nephrology Nurse (CNN) or Certified Case Manager (CCM) preferredDemonstrated knowledge and understanding of data and managing to clinical, financial, and patient satisfaction outcomesDemonstrated experience and effectiveness in change agent roleDemonstrated knowledge and understanding of CQI techniquesPrevious experience in healthcare performance coaching requiredCurrent CPR certification requiredAbility to modify personal practice patterns to adapt to new / electronic processes and increased productivity expectations as it pertains to Capella implementationCurrent driver's license in state employed with positive driving record and able to meet requirements of insurance coverage requiredBasic computer skills and proficiency in MS Word and Outlook requiredFunctional proficiency with DaVita specific clinical software programs, including Capella, required within 90 days of employmentHome office with internet connectivity at minimum of 1MB upload and 1MB download speed requiredJoin us as we pursue our vision "To Build the Greatest Healthcare Community the World has Ever Seen."Why wait?
