RN – Case Management
Seven Healthcare is seeking an RN Case Manager specializing in Case Management for a travel contract in Clanton, AL. This role involves working onsite and engaging directly with patients in various settings. The candidate will be responsible for providing comprehensive assessments and coordinated care for high-risk members.

Key responsibilities of the RN Case Manager include:
- Perform comprehensive assessment of high-risk members
- Collaborate with primary care providers for comprehensive care plans
- Identify and reassess appropriate levels of care
- Coordinate non-medical support for treatment compliance
- Facilitate care transitions with multidisciplinary teams
- Engage specialty resources to achieve optimal outcomes
- Document clinical, functional, and fiscal outcomes accurately
- Promote health and illness prevention strategies
- Act as a patient advocate, ensuring privacy and confidentiality
Requirements of the RN Case Manager include:
- Associates or Bachelor's Degree in Nursing or related field
- Current, unrestricted RN license in Alabama
- Case Management certification
- Ability to analyze complex medical information
- Experience with home healthcare, OASIS, and HCHB
