Backend Engineer

About the Role Join a fast-growing, early-stage healthtech AI company as one of its first backend engineers. The company builds AI voice and browser agents that automate the most painful administrative workflows for independent specialty clinics — including referral processing, scheduling, and insurance prior authorizations — and is already running in live production environments across Northern California. You'll own the core services powering these agents, work directly with the founding team, and ship into real customer environments every week. This is a high-ownership, high-impact role at a company where your work directly affects how clinicians spend their time. What You'll Do Design and build core backend services and APIs that orchestrate AI voice and browser agents Build integrations with EHRs, scheduling systems, and insurance/payer portals Own reliability, observability, and performance of agent workflows in production Partner with the founders on architecture, tradeoffs, and product direction Help shape engineering culture, hiring, and on-call practices as the team scales What We're Looking For Required: 3+ years building production backend systems in Python and/or TypeScript/Node.js Strong fundamentals in system design, databases (PostgreSQL), and distributed systems Comfort with queues and event-driven architectures Experience with cloud infrastructure (AWS or GCP) and modern observability tooling Strong product instinct and ownership mindset — able to scope, ship, and iterate independently Current work authorization to work remotely from a LATAM-based country or the United States (no visa sponsorship available) Nice to Have: Experience with LLM agents or deployed AI models in production Background in voice technologies (telephony, ASR/TTS) Experience with browser automation Familiarity with healthcare/HIPAA-compliant environments Compensation & Benefits Salary: $70,000 – $100,000 USD/year (LATAM-based) or up to $150,000 USD/year (US-based), depending on experience Founding-team equity Direct access to founders and real customers from day one Location Fully remote — open to candidates in LATAM (with overlap to US Pacific hours) or in the United States No visa sponsorship available; candidates must have current work authorization
