Wood River, IL | Skilled Nursing Facility / Long-Term Care — Float position covering multiple nearby facilities within approximately 10 miles of each other

Key Responsibilities
- Conduct thorough evaluations of patients' physical abilities and limitations
- Develop and implement personalized treatment plans based on evaluation findings
- Utilize a variety of therapeutic techniques and modalities to facilitate patient recovery
- Collaborate with interdisciplinary teams, including physicians, nurses, and occupational therapists, to optimize patient care
- Educate patients and their families on rehabilitation goals, techniques, and home exercise programs
- Monitor patient progress, adjust treatment plans as necessary, and maintain accurate and timely documentation of patient assessments and treatments
Required Qualifications
- Valid and unrestricted Physical Therapist license in Illinois
- Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program
- Strong interpersonal and communication skills
- Ability to work effectively in a team-oriented environment
- Commitment to providing high-quality, patient-centered care
Preferred Experience / Skills
- Experience in a skilled nursing or long-term care setting
- Familiarity with regulatory requirements and industry best practices in rehabilitation
