Job Description
Title: LPN - Long Term Care

Schedule: All contracts must run through the holiday season and HCP must be available for full shift orientation.

Assignment Details:
- Contract Length: 27 weeks
- Guaranteed Hours: 24 hours per week

Requirements:
- NY LPN Licensure
- BLS (AHA or RQI)
- 1 year of experience as LPN

Roles & Responsibilities:
- Provide direct and indirect resident care under the supervision of licensed nursing personnel, ensuring patient needs are met safely and effectively.
- Assist residents with activities of daily living (ADLs) including hygiene, grooming, dressing, and mobility.
- Maintain a safe and clean environment for residents and respond promptly to call lights and alarms.
- Support residents' nutritional needs and preferences, including meal delivery and feeding assistance.
- Participate in rehabilitation activities and monitor residents for changes in physical and emotional status, reporting concerns to nursing staff as needed.

Compliance / Notes:
- Occupational exposure to blood borne pathogens is reasonably anticipated.
- Required tasks involve potential contact with blood, body fluids, or tissues.
