Title
Physical Therapist - Physical Therapy

Assignment Details

Requirements
- Current State License as a Physical Therapist
- Proof of current CPR certification
- Current driver's license with reliable transportation and insurance
- 1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting
- Previous home care experience (preferred)
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program

Roles & Responsibilities
- Perform clinical assessments to identify patient needs and determine appropriateness for physical therapy.
- Document and prepare a Plan of Care (POC), progress notes, and other clinical records based on ongoing assessment data.
- Plan and provide treatment for neuromuscular, neurological, and orthopedic disorders based on medical referrals.
- Instruct patients and families in transfer techniques and the use of therapeutic equipment to ensure safety and proper usage.
- Provide patient and family education regarding therapeutic procedures, tailored to the patient's age and condition.
- Work collaboratively with other team members to ensure consistent and appropriate patient care.
- Apply infection control and universal precautions in all patient care activities.
