Compensation: $120,000 to $160,000+ Total Compensation (Base Salary + Productivity Incentive Program) + Mileage Reimbursement

As part of our growing medical group, you'll provide on-site primary care throughout assisted living, memory care, and long-term care communities across the Twin Cities Metro. You'll play a central role in clinical decision-making, helping coordinate care, manage complex conditions, and guide treatment plans that directly affect patient outcomes.

What You'll Be Doing

• Provide on-site primary care and rounding services throughout assisted living, memory care, and long-term care communities.

• Perform comprehensive assessments, chronic disease management, and preventative care.

• Develop and adjust individualized treatment plans.

• Collaborate with nurses, therapists, caregivers, physicians, and facility leadership.

• Support transitions of care and help reduce avoidable emergency room visits and hospitalizations.

• Serve as a key clinical partner for patients, families, and community staff.

Qualifications

• Active Minnesota Physician Assistant (PA-C) license in good standing.

• Experience in geriatrics, family medicine, internal medicine, post-acute care, hospital medicine, or related settings preferred.

• Strong communication skills and confidence working independently.

• Valid driver's license and reliable transportation.
