Community Care is hiring a Case Manager/Care Manager to join our Family Care Program team in Kenosha County. This full-time role offers a Monday–Friday schedule (8:00 AM–4:30 PM) and a $5,000 sign-on bonus. You will collaborate with an RN Care Manager to assess members, develop care plans, and coordinate services with a focus on helping seniors and adults with disabilities live independently.
