LPN Continuing Care

As a continuing care LPN, providing top-notch patient care is paramount. Under the direction of an RN, mid-level provider, or physician, you will be responsible for assessing, planning, implementing, and evaluating patient care. You will be a vital part of a team, making a real difference in the lives of patients and their families. Essential functions - roles and responsibilities include:

• Collect and monitor assessment data.
• Report physiological, psychological, sociocultural, and spiritual needs of patients and families.
• Provide direct patient care under the direction of an RN, mid-level provider, or physician.
• Inform patients and families about treatments and medications being received.
• Document care provided via computer or written patient records.
• Communicate and collaborate with other members of the healthcare team.
• Clean the patient's environment, work area, and equipment.
• Document patient chargeable items according to standards.
• Participate in quality monitoring activities.
• Demonstrate a high standard of moral and ethical behavior.
• Advocate for privacy, confidentiality, and security of patient and staff data.
• Actively participate in safety initiatives and risk-mitigating measures.
• Perform other duties as assigned.
