Position Overview We are looking for a dynamic and autonomous Nurse Practitioner to serve the Phoenix location. The successful candidate will play a crucial role in providing direct patient care, developing chronic care plans, and collaborating with referring providers to optimize patient outcomes. Responsibilities Clinical Leadership: Provide direct patient care through all available channels and products. Conduct comprehensive assessments, develop treatment plans, and implement evidence-based care practices. Monitor and evaluate patient progress, adjusting treatment plans as needed. Collaborate with interdisciplinary teams to ensure seamless patient care coordination. Chronic Wound Care Planning and Management: Develop personalized treatment plans for patients with wounds, focusing on long-term management and prevention of complications. Educate patients and families on self-management strategies and lifestyle modifications. Communication and Collaboration: Establish and maintain effective communication channels with referring providers to ensure continuity of care. Collaborate with healthcare professionals, including physicians, nurses, and other specialists, to optimize patient care. Qualifications Current and unrestricted Nurse Practitioner license in Arizona. Certification as a Nurse Practitioner with a specialization in wound care management is preferred.
