Ellicott City Healthcare Center is seeking a compassionate, dedicated, and licensed Social Worker to join our interdisciplinary care team. This role is responsible for supporting residents and their families by providing psychosocial assessments, care planning, counseling, discharge planning, and advocacy services that enhance quality of life and promote resident well-being.

The ideal candidate is a caring professional who is passionate about helping individuals navigate healthcare challenges while ensuring compliance with federal, state, and facility regulations.

The Licensed Social Worker serves as a key member of the healthcare team, addressing the emotional, social, and psychological needs of residents. This position collaborates closely with nursing, therapy, admissions, and medical staff to develop individualized care plans and facilitate positive resident outcomes.

• Conduct psychosocial assessments for residents upon admission and as needed.

• Develop, implement, and maintain individualized care plans.

• Provide counseling, emotional support, and crisis intervention for residents and families.

• Assist with discharge planning, care transitions, and community resource coordination.

• Advocate for resident rights and promote resident-centered care.

• Participate in interdisciplinary team meetings and care conferences.

• Educate residents and families regarding available services, benefits, and support programs.

• Assist residents and families with advance directives and healthcare decision-making when appropriate.

• Document assessments, interventions, and progress notes accurately and timely.

• Ensure compliance with CMS, state, and facility regulations.

• Address concerns, grievances, and social service needs promptly and professionally.

• Maintain positive relationships with residents, families, referral sources, and community agencies.

• Current Maryland Social Work license (LSW, LMSW, or equivalent as required by state regulations).

• Bachelor's or Master's degree in Social Work from an accredited program.

• Minimum one (1) year of social work experience in a healthcare, long-term care, rehabilitation, skilled nursing, or related setting.

• Knowledge of discharge planning, care coordination, and community resources.

• Strong interpersonal, communication, and problem-solving skills.

• Ability to work collaboratively in a multidisciplinary environment.

• Experience in skilled nursing, rehabilitation, or long-term care.

• Knowledge of Medicare, Medicaid, and healthcare regulations.

• Experience working with geriatric populations.
