Job Description

NorthBay Medical Center is seeking an experienced RN Case Manager to coordinate patient care throughout the hospital stay and ensure safe, timely, and cost-effective discharge planning. The Case Manager will work closely with physicians, nursing staff, social workers, and other healthcare professionals to develop individualized care plans, perform utilization review, and connect patients with appropriate post-acute services. Candidates should be able to work independently in a fast-paced acute care environment while maintaining compliance with CMS and hospital guidelines.

Requirements

• BSN required.
• Active California RN License .
• Minimum 2 years of acute care RN experience .
• Minimum 2 years of inpatient Case Management experience .
• Travel nursing and/or California hospital experience preferred.
• Experience with Cerner and Epic preferred.
• CCM or ACM certification preferred.
• ED experience is a plus.
