Job Title: LPN Profession: Licensed Practical Nurse Specialty: Staff Augmentation Duration: 40 hours Shift: Multiple Hours per Shift: 12 hours (6:00 AM - 6:30 PM) Experience: Minimum years of experience required License: Current LPN License Certifications: Basic Life Support (BLS) certification preferred Must-Have: - Knowledge of gerontological/geriatric nursing theory and practice - Strong observational and communication skills - Ability to follow nursing regulations - Interpersonal skills to establish effective work relationships

Description: Schedule: Full Time or PRN available. Block scheduling rotates with two consecutive days off and every other weekend. Administer medication and perform treatments as ordered by the physician and within the scope of license. Medications may be in a variety of forms, including oral, enteral, parenteral, and suppository. Treatments may include sterile and aseptic wound cleaning and dressing, postural drainage, tracheostomy, gastrostomy, jejunostomy care, and catheter care. Observe and report the effectiveness of medications and treatments. Ensure an adequate amount of supplies and medication are available. Receive and deliver various information regarding the resident's medical, social, functional, and other needs. Participate in the receiving and giving of shift reports. Communicate with residents, family, and staff as necessary to ensure appropriate care from the interdisciplinary team. Complete the initial admission nursing assessment tool for newly admitted residents. Assist with the development of Minimum Data Set (MDS) assessments, ongoing quarterly reviews, and care plans. Maintain appropriate clinical and administrative documentation regarding diagnosis, observations, treatment, and summary of resident progress. Participate in Quality Assurance and Performance Improvement (QAPI) initiatives. Attend and actively participate in staff meetings. Skills in observing and reporting behavior and progress of residents/patients are essential. Ability to apply and interpret state and agency laws and regulations governing geriatric nursing practices. Assist in the development and implementation of patient care plans within a team nursing concept. Oral and written communication skills are important. Ability to listen, observe, assess, and respond to medical emergencies is required. Evaluate, analyze, make independent decisions, and follow up on client health problems. Respond to medical issues and inquiries professionally. Establish and maintain effective work relationships.
