RN Care Coordinator

Boston Medical Center

Utilizing a collaborative process, the care manager will assess, plan, implement, monitor, and evaluate the options and services required to meet an individual’s health needs, using communication and available resources to promote quality, cost-effective outcomes. The care manager helps identify appropriate providers and facilities throughout the continuum of services while ensuring that available resources are being used in a timely and cost-effective manner to obtain optimum value for both the patient and the reimbursement source. This care manager is dedicated to handling increased volume and supporting the overall care management process and the department. Position RN Case Manager Schedule 40 Hours (Days / 5x 8‑Hour Shifts / Every 3rd Weekend) Education Diploma, ASN or BSN (preferred) Ability to obtain BSN within 4 years Certificates, Licenses, Registrations Licensed to practice as a Registered Nurse in the Commonwealth of Massachusetts (required) Certified Case Manager (CCM) or related certification (preferred) BLS (Basic Life Support) (required) Experience A minimum of 3–5 years recent clinical experience (required) A minimum of 1–3 years recent acute inpatient care management experience (required) Knowledge and Skills Strong communication, collaboration, and problem‑solving skills Knowledge of utilization review, discharge planning, managed care, and community resources Extensive background and knowledge of current medical/surgical patterns of practice Medical terminology and standard medical abbreviations Managed care concepts and various health‑care delivery systems Assessment methods for levels of physical/mental impairment and understanding of illness characteristics Confidentiality issues and legal and ethical issues related to them ICD‑9 and CPT nomenclature and ability to interpret and convert this information Knowledge of benefits and various plan contracts, eligibility issues and requirements Role and functional responsibilities of case management professionals and program philosophy within managed care Excellent interpersonal, verbal and written communication and negotiation skills Strong analytical, data management and PC skills Compensation Range $85,176.00 – $196,747.20 (based on minimum job qualifications) Benefits Medical, dental, vision, pharmacy coverage Contract increases Flexible Spending Accounts 403(b) savings match Earned time cash out Paid time off Career advancement opportunities Resources to support employee and family wellbeing Equal Opportunity Employer Boston Medical Center is an equal employment/affirmative action employer.
