RN/LPN Wellness Nurse

Maplewood Brewster LLC, a senior living community specializing in assisted living and memory care in Brewster, MA, is seeking a compassionate and clinically skilled RN/LPN Wellness Nurse. In this role, you will play a central part in supporting residents’ health, independence, and overall quality of life within a warm, engaging, and resident-centered environment. As an RN/LPN Wellness Nurse, you will provide direct, hands‑on nursing care to residents, including ongoing health assessments, medication administration, treatments, and coordination of individualized wellness plans. You will monitor changes in residents’ conditions, respond promptly to clinical needs, and communicate effectively with physicians, families, and the interdisciplinary care team to ensure that each resident receives tailored, clinically appropriate care. Your daily work will include documenting care and observations in electronic medical records, following infection control and safety protocols, participating in care plan meetings, and supporting caregivers with clinical guidance. Working closely with residents living with memory impairment, you will use patience, empathy, and specialized approaches to support dignity, comfort, and emotional well‑being. You will help create an environment that minimizes behavioral distress and maximizes engagement, safety, and meaningful daily experiences. Collaboration is essential in this role: you will partner with care managers, activities staff, and other team members to align clinical care with residents’ preferences, functional abilities, and life histories.
