About the Position

Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 (08:00 - 17:00) Experience: Minimum of 1 full year of recent adult home health or hospice experience depending on specialty License: Must have an active license Certifications: CPR and BLS required Must-Have: Proficiency in Homecare Homebase (HCHB) and OASIS Description: This is a home health assignment. The RN will provide home visits and attend periodic office meetings. The average daily caseload is 5 to 8 patients. Weekly productivity expectations are 30 points. Visit types, documentation, travel, and other activities have different point values. Contractors typically see 5 to 10 patients per day, totaling approximately 6 to 8 points. There are no guaranteed hours. If the patient census is low, contact the Account Manager about floating to a nearby clinic for additional hours. Case types may include start of care, wound care, follow-up visits, and other adult home health or hospice services. Must be willing to travel up to 75 miles from the home clinic. Reliable transportation, a current driver's license, and agency-required liability insurance are required. Mileage is reimbursed at the IRS standard rate. Travel between patient homes is reimbursable. Travel to the first patient and from the last patient is not billable. Local candidates are accepted at the same rate. Pending licenses are not accepted; an active license must be in hand. Must be proficient in Homecare Homebase (HCHB) and have used it within the past six months. Clinicians with prior HCHB experience outside the six-month window may be considered but must complete retraining during compliance. All documentation must be completed within 24 hours of the appointment with the patient. Delayed and incomplete documentation that impacts billing and follow-up patient care will result in cancellation. Confirmed understanding that there are no guaranteed hours with this contract. The clinician must hit weekly productivity points of 30, which equates to 40 hours per week. All overtime must be pre-approved. If the clinician is not receiving enough patients each week, they will have the opportunity to float to another nearby clinic to pick up patients.
