Clinical Pharmacist - Inpatient & Ambulatory Care (Sign-On)

Carilion Roanoke Memorial Hospital / Crystal Spring Tower – Roanoke, Virginia $30,000 Sign-On Bonus with Up to $10,000 for Relocation Clinical Staff Pharmacist – Day / Evening (Main Campus) Provide direct patient care across inpatient units (medicine, surgery, critical care, specialty services). Order verification, pharmacokinetic dosing, antimicrobial stewardship support, discharge counseling, and multidisciplinary rounds participation. Rotate day and evening shifts with shared weekends and holiday coverage. Clinical Staff Pharmacist – Midnights (Main Campus) Support 24/7 operations for the regional referral and Level 1 trauma center. Prospective order review and verification, sterile compounding oversight, emergent medication support for ED and critical care. Collaboration with nursing and medical staff to ensure safe, timely medication therapy. Rotate through midnight shifts with focus on autonomy and efficient workflow. Clinical Staff Pharmacist – Midnights, 7-on / 7-off (Main Campus) Block schedule of 7 days on and 7 days off in a high‑acuity inpatient environment. Comprehensive overnight pharmacy services: order verification, code and rapid response support, TPN and anticoagulation management. Troubleshoot medication‑related issues across the hospital. Clinical Staff Pharmacist – Day / Evening (Main Campus) Support medication use across assigned units. Answer drug information questions and collaborate with providers on therapy optimization. Participate in quality and safety initiatives. Shift rotation between days and evenings with shared weekend and holiday responsibilities. Clinical Staff Pharmacist – Crystal Spring Tower, Day / Evening Support ambulatory and outpatient services in the tower setting. Verify orders, support infusion and procedural areas, and coordinate transitions of care. Work closely with physicians and nurses in clinic‑adjacent environments. Day and evening shifts with occasional weekend coverage as needed. Clinical Staff Pharmacist – Crystal Spring Tower, Nights Provide overnight coverage for tower‑based services. Verify medication orders and support evening and night infusion or procedural needs. Collaborate with main campus pharmacy and clinical teams. Work more independently while remaining integrated into the larger pharmacy team. Experience: Pharmacy Practice Residency or equivalent Hospital experience is preferred. Licensure, certification, and/or registration: License to practice Pharmacy in the Commonwealth of Virginia required within 90 days of hire. During the first 90 days of employment and the licensure application process, pharmacists without a valid license may not perform duties restricted to a licensed pharmacist until the license is obtained. If the license is not obtained within 90 days of employment, the employee may be subject to separation from Carilion Clinic. Other Minimum Qualifications: Must possess good leadership, communication, interpersonal and teamwork skills. Must be able to effectively budget time, work efficiently and accurately, and follow procedures with minimum supervision. Excellent critical thinking and analytical skills.
