• Shift: Monday - Friday, 8-hour days (08:00 17:00), flexible schedule with weekends and nights possible
• Hours/Week: 40
• Employment Type: Contract (month-to-month)
• License/Certs: Active Oregon Medical License, enrolled/Credentialed in Medicard
• Experience: 1+ years preferred in primary care or family practice

• Pay rate: $0.01/hour

What You'll Do:

• Conduct annual risk assessments and chronic condition reviews for pediatric and adult patients
• Deliver telehealth consultations (12-14 patients/day) and in-home visits (6-7 visits/day), based on schedule and preference
• Perform assessment-only duties; no treatment or prescribing required
• Escalate urgent findings to patients' PCPs promptly
• Travel within the Milwaukee area; mileage reimbursed at IRS standard rate
• Maintain documentation and coordinate care via telehealth and home visits

What You Need:

• Active Oregon Medical License
• Enrollment in Medicard (must be highlighted during submission)
• 1+ years of family practice or primary care experience preferred
• Comfortable seeing both pediatric and adult patients
• Strong assessment and communication skills
• Ability to work independently and manage a flexible schedule
