Hospice RN Case Manager: Home-Based Care Leader

Providence at Home with Compassus in Seattle area seeks an experienced Hospice Registered Nurse (RN) to assess patients in home settings and deliver compassionate, patient-centered care. You will collaborate with the Interdisciplinary Group (IDG) to plan care, monitor patient status, and adjust plans as needs evolve, ensuring comfort and dignity at end of life. Qualifications include an active RN license, 1 year of hospice or home health experience preferred, and an accredited nursing degree #J-18808-Ljbffr Compassus
