Position Overview We are seeking a compassionate, mission-driven Psychiatric Nurse Practitioner (PMHNP) to provide high-quality psychiatric care to adults with serious mental health conditions as part of a multidisciplinary, community-focused treatment team. This role delivers services directly in the community and emphasizes individualized, recovery-oriented, and person-centered care for individuals with complex behavioral health needs, including co-occurring disorders. The Psychiatric Nurse Practitioner plays a central role in psychiatric assessment, diagnosis, medication management, and ongoing treatment. Services are flexible and responsive to participant needs, with individuals engaging with a psychiatric provider at least once per month in community or office-based settings. Core Responsibilities Conduct comprehensive psychiatric evaluations and establish accurate diagnoses Develop, implement, and adjust individualized treatment plans in collaboration with the interdisciplinary team Prescribe and manage psychotropic medications in accordance with best practices and program guidelines Provide ongoing follow-up visits to monitor symptoms, treatment response, and overall progress Collaborate closely with team members including nursing, social services, substance use specialists, vocational staff, and peer professionals Participate in team meetings, case conferences, and shared clinical decision-making Serve as a psychiatric resource by offering consultation and guidance on complex cases Assist with crisis intervention, stabilization, and urgent psychiatric needs as required Complete clinical documentation accurately, timely, and in compliance with regulatory standards Stay current with developments in psychiatric care, community mental health models, and evidence-informed practices Program & Organizational Support Contribute to quality improvement initiatives and monitoring of clinical outcomes Participate in required trainings, supervision, and professional development activities Perform additional duties as needed to support program operations and participant care Required Knowledge, Skills & Attributes Strong clinical expertise in the assessment and treatment of serious mental illness, including co-occurring conditions Knowledge of regulatory and compliance requirements for community mental health services Commitment to person-centered, trauma-informed, and culturally responsive care Ability to work effectively within a team-based, multidisciplinary care model Excellent verbal and written communication skills, particularly in community-based settings High standards of professionalism, ethical practice, and accountability Proficiency with electronic health records and standard office software Education & Experience Licensed and board-certified Psychiatric Nurse Practitioner (PMHNP) Minimum of six (6) months’ experience working with individuals diagnosed with serious mental illness Prior experience in community-based behavioral health, assertive treatment models, or similar programs strongly preferred Flexibility to deliver services across community and office settings, with adaptable scheduling #J-18808-Ljbffr Medix™
